Build Confidence in Accurate Documentation and PDPM Success

Accurate documentation is the backbone of MDS coding and the Patient-Driven Payment Model (PDPM) for Medicare or State Medicaid methodology. This program provides the fundamental education the interdisciplinary team (IDT) needs to create sound supportive documentation to support each element used in PDPM.

PDPM documentation by an IDT member

PDPM Documentation Essentials for the IDT is a ten-part on-demand workshop, exploring how proper documentation impacts PDPM case-mix categories. Each module provides practical tips for frontline documentation across disciplines—helping the care team work together to ensure coding accuracy, regulatory compliance, and the best outcomes for residents and facilities alike.

Following this workshop, you will be able to:

  • Recognize the importance of accurate documentation in supporting MDS coding for PDPM classification and reimbursement accuracy
  • Describe situations when documentation of scripted interview is recommended in the medical record
  • Identify MDS items that impact PDPM and the documentation to support them including section GG, primary diagnosis selection, and weight loss calculation
  • Identify how documentation accuracy affects clinical care, MDS accuracy, quality programs, regulatory compliance, and reimbursement
